This is a qualified life event. When you experience a qualifying life event, such as adding a new child to your family, you have 30 days to make benefit changes that are consistent with your life change. (For example: If you have a baby, you can add your new child to your medical plan.) Contact Human Resources to get the process started if you need to make a change. If you don't meet the deadline, you'll have to wait until the next Annual Enrollment to make any changes to your benefits.

  • Always make sure your personal information stays current. Contact Human Resources if you need to make changes to your name, address, phone number, marital status, tax withholding information, or emergency contact information.
  • If you’re changing your name, get a new Social Security card.
  • Order new business cards and new corporate credit card, if necessary.
  • Compare your and your new spouse’s coverage options before making your benefit choices.
  • Change or update your benefits coverage within 30 days of your marriage. You must provide a copy of your marriage certificate to change any benefit. You may enroll yourself, your new spouse, and any eligible dependent children. If you want to add your new spouse’s dependents, you must provide a birth certificate for child(ren) with a different last name. You may cancel your coverage if you enroll in your new spouse’s coverage. If you do, your Siegwerk coverage will end at the end of the month in which you cancel.
  • Consider starting or increasing contributions to a Health Care Flexible Spending Account (FSA) or a Health Savings Account (HSA).
  • Consider adding voluntary spouse life insurance or updating your life insurance beneficiary information.
  • Update your retirement plan beneficiary and update your name and address with your retirement plan provider, if necessary.

  • Enroll in benefits if you had coverage through your ex-spouse or remove your ex-spouse (and any ex-step children) if they were on your Siegwerk benefits. Changes must be made within 30 days of the official date of divorce. You must present a Copy of your Divorce Decree for all benefit changes.
  • Consider any necessary changes to your Flexible Spending Account (FSA) or Health Savings Account (HSA) contributions.
  • Update your supplemental life insurance coverage and beneficiary information.
  • Update your retirement plan beneficiary and update your name and address with your retirement plan provider, if necessary.
  • Update your will.

  • Contact Human Resources to confirm the company’s bereavement and personal leave policies.
  • Contact Human Resources with any questions about the deceased’s benefits coverage. Changes, such as removing the deceased family member from coverage, must be made within 30 days.
  • Contact the Social Security Administration to end benefits received by the deceased (if disabled or retired) and to determine if benefits are available to the surviving spouse and dependent children.
  • Submit life insurance claims if you are the beneficiary. You must present a copy of the official Death Certificate for claims processing.
  • Update your life insurance beneficiary, if necessary.
  • Update your retirement plan beneficiary, if necessary.
  • Update your will, if necessary.

  • Notify your manager and Human Resources about your intent to leave the company.
  • Make decisions about your ongoing benefits coverage. You can expect to receive COBRA information mailed to you at home after your benefits coverage ends. Continuation of your Siegwerk benefits through COBRA is available for up to 18 months from the last day of the month in which you terminate. You pay the full cost of COBRA continuation without any subsidy from the company. Cost information will be included in the mailing you receive.
  • If you elect COBRA, you may continue using your Flexible Spending Account. Contact Health Equity for information about submitting claims for reimbursement.
  • Convert supplemental life insurance to an individual policy, if desired.
  • Contact your retirement plan administrator to learn about your options. Ask about the process for loan repayment, if applicable, and determine if you are eligible to keep your vested balance with the plan.

This is a qualified life event. After you’ve left the company and are no longer eligible for Siegwerk benefits, you are eligible to enroll in other coverage, such as COBRA coverage or your spouse’s benefits. Ask your spouse’s employer for their enrollment process and deadline.

  • Check with Health Equity about continuing Flexible Spending Account (FSA) coverage for eligible expenses.

  • Contact Human Resources if you need to update your tax withholding information as a result of your dependent’s status change.
